Forms

Below are the necessary forms for your camper. Returning campers need to fill out and return health forms, horseback riding waiver, and internet waiver by May 15. New campers need to fill out and return health forms, horseback riding waiver, internet waiver, and confidential form by May 15. Forms can be mailed to Tamahay or sent electronically. We recommend using out gear list for packing purposes.
